Thank you for entrusting your pet’s travel needs to Highlands Veterinary Center. We look forward to assisting you and your pet! The Health Certificate process can be lengthy and in depth and we ask for your patience during this time.

If your pet is traveling internationally, please be aware that there may be additional testing and documentation that is required by certain countries. For further information please visit the USDA APHIS website https://www.aphis.usda.gov/pet-travel.

What to expect during the Health Certificate process:

  • Submit all your pet’s medical records, including: signed rabies certificates, microchip information (including date of implantation), and vaccine information. Please submit them as an attachment via e-mail to hvcreception1@gmail.com. (**Failure to provide medical records/vaccine history may prevent /impede our ability to process the health certificate timely or your ability to travel with your pet at your desired departure time.**)
  • Schedule a date for your pet’s exam for their Health Certificate. This must be scheduled within 10 days of travel, as your pet’s Health Certificate is only valid for 10 days. We recommend scheduling your appointment at the earliest available date.
  • Following your pet’s exam, the Health Certificate will then be submitted electronically to the USDA for approval and endorsement. Your pet will not be permitted to travel abroad without this proper documentation.
  • Once the Health Certificate has been submitted to the USDA it is officially in the government’s control to be endorsed. We at Highlands Veterinary Center will do our best to submit your Health Certificate with expedience and haste, however, while the date of travel is listed in the submitted documentation, please note that speed in which the Health Certificate is endorsed is completely out of our purview and control.
  • Following the endorsement of your Health Certificate by the USDA, your pet’s Health Certificate will then be shipped overnight back to Highlands Veterinary Center via FedEx or UPS.
  • Once your Health Certificate has been received by Highlands Veterinary Center you will receive a call to pick it up. Your pet will not be permitted to fly or enter into another country without the proper documentation (i.e. Health Certificate and Rabies certificates).
  • Please Note: Pet travel services with Highlands Veterinary Center include exportation from the US ONLY. Highlands Veterinary Center does not assist with the importation or return of pets to the US. Returning to the US is the responsibility of the pet owner. For guidance on returning to the US, please contact the USDA, CDC, or a local veterinarian at your destination for assistance.

The health certificate process can be lengthy and quite in depth, so to help us best assist you, we ask that you complete this form. The information you provide will be used to complete your pet’s official health certificate, which will then be sent to the USDA for approval and endorsement, so please ensure that it is complete and accurate.

Your pet will be scanned for a microchip during your visit. If your pet does not have an ISO compliant microchip, we strongly recommend one as the CDC requires it for re-entry to the US.
Microchip must be implanted prior to the first Rabies Vaccine.
